Hello, Sir Anguish,<p>Whenever you enter a room or an area, before the enemies emerge, the game checks to see which enemies they are. If you're lucky, the game will switch one of those enemies with a rare version of itself, depending on the enemy. (Some enemies don't have rare counterparts). The Al Rappy is the rare version of the Rag Rappy. Since these enemies are rare, you can usually expect a rare — or at least useful — item from them after slaying them. (In your case, a Scape Doll — which will really come in handy later on in the game, once you start getting into the higher modes of difficulty, especially Ultimate.)<p>Anyway, I hope that answers your question.
